Session Request - Form

A few quick but important notes:

• I do not meet with anonymous people due to the sensitive nature of the services that are being offered. If you can't trust me with your real name, I don't want to be in a room with you alone.
  This information is COMMON and STANDARD to be requested from reputable professionals, along with a monetary deposit.
  
  If you are unwilling to share materials that verify your identity, you must have a verifiable reference from the past 4 months. That means a provider with an active ad, and a website or social media that is current. Please check with your contact first before sharing their information with me. This can take a couple days to sort out and is not ideal for short notice meetings unless they are fast to respond.

  
  To complete this booking request, please attach in your email the following:

  1) A selfie holding of your ID Please no sunglasses. Fully hats welcome. I must be able to see your upper body.
  2) A stand-alone photo of your ID
  
  OR
  
  3) Selfie, face fully visible, no hats or glasses.
  
  4) Two verifiable references of companions you have seen within the last 6 months
  You MUST ask your reference first if you can share their information with me
  When they give you the OK, include the following:
  -Name
  - Website
  - Link to ad
  -Email address
  -When did you see this provider?
  
  

• This form is to streamline booking and verify that you are who you say that you are and no other reason.
  
  

• Your information will never be shared or retained, responses are deleted upon verification.
